HHH Choices, a voluntary long-term managed care program approved by the New York State Department of Health and available to Bronx residents covered by Medicaid, provides many services for the elderly and people who are physically challenged, chronically ill, or disabled. The HHH Choices Plan Nurse Care Manager oversees all of the care provided to members. Membership continues even if members become residents of Hebrew Hospital Home or another nursing home, and the HHH Choices Plan Nurse Care Manger continues to oversee the services that members receive.

Services include case management, professional nursing care, home care services, transportation, emergency response systems, adult day care, social day care, dental care, podiatry, vision/hearing services, medication, durable medical equipment, home equipment, medical and surgical supplies, nutritional counseling, and social and environmental assistance.
