The Long-Term Home Health Care Program services the Bronx and provides skilled, high caliber nursing and health care services to persons ordinarily eligible for nursing home placement, enabling them to remain in the community and in control of their lives. It provides an opportunity for the elderly and chronically ill to remain at home while decreasing the burdens placed on their families. Professional caregivers include licensed registered nurses and rehabilitative therapists, certified social workers and nutritionists, and aide services. Medicaid, Medicare, or private payment is accepted.
